FLEX CREDIT APPROVAL & REPORTING FORMS

1. To present a professional development activity during fall or spring in-service week, click HERE Report this presentation on the e-form mailed to you after in-service week.
2. Attend a scheduled activity during fall or spring in-service week. A "scheduled" activity is one that appears in the fall or spring in-service week printed program. The reporting e-form will be emailed to you after in-service week ends.
3. Attend a professional development activity that does not appear in the fall or spring in-service week printed program. First attend the activity, then select one of the forms below. They ask the same questions, only their format differs.

4. Undertake and complete a self-directed, individual activity.
	Before beginning the activity, complete an approval form and ask your division flex representative to sign it. Either the flex rep or you should send the approval form to the Office of Instruction, AGB 121. Obtain the form at the link below: http://www.saddleback.edu/gov/senate/approval.doc
	After approval, complete the inmdividual activity. Then select one of the reporting forms below to report your results. The forms ask the same questions, only their format differs.
